The preliminary report by the Aircraft Accident Investigation Bureau revealed that the twin engines of the Air India flight shut down mid-air due to fuel supply being cut off, leading to a rapid descent and crash outside the airport perimeter.
There was confusion between the pilots in the cockpit, with one pilot asking why the fuel was cut off, although neither had acknowledged turning off the switches. Efforts to restart the engines were unsuccessful.
The aircraft hit its top airspeed and moments later, the fuel cut off switches moved from 'RUN' to 'CUTOFF' position. A backup power device deployed after a total loss of power was indicated by CCTV footage.
The investigation also found no significant bird activity near the flight path and that the crew was medically fit and experienced. Air India had not inspected its planes despite an advisory about a fuel-control switch locking feature.
