A cybersecurity firm ran a customer survey offering iPads as prizes and stored them in a secure safe, only to find them stolen a year later.
The theft led to investigations that revealed the company's Head of Legal, an ex-convict, had taken the iPads.
Following this incident, the company implemented mandatory background checks on all staff.
A security breach was discovered when an employee found out that the background check website, developed by an ex-convict who was a used car salesman, had weak security protocols and exposed employee information.
